Misc fixes and tweaks
- fix failure to stop the countdown or reset the icon when the page is
  refreshed by the user or automated challenge solvers
- convert icons text to paths to avoid dependency on system fonts
- increase max countdown to 15s
- increase the min countdown for 403 requests to 10s to give
  automated challenge solvers time to complete
